Woodville ISD Wellness Policy

If your child is diagnosed with a medical illness it is important an action plan is developed by you and your doctor specifically to manage the illness. It will provide guidance for the nurse on proper use of medicines, triggers that need to be avoided, how to respond to worsening symptoms, when to call the doctor, and when to seek emergency care. It is important to have this on file so the best care can be provided for your child. Below is a list of our action plans available for you to print off and take to your MD to have completed or you may also obtain a copy from your campus nurse.

• Anaphylaxis

• Asthma

• Diabetes

• Hypoglycemia

• Seizure

• No Treatment Needed Agreement

Woodville ISD does not provide any medications for students. All medications administered at school should be brought to school by the parent/ guardian with a signed Medication Consent Form. Below is a link to our form. You may also obtain a copy from your campus nurse.
